Collecting responses from patients
Patients automatically receive surveys and notifications, depending on your practice and patient survey settings
You can manually send a survey at any time by clicking "Request" in the Patient Settings
If the desired survey isn't listed, click "Add Survey" then "Request"
Patients complete surveys through the Osmind app or patient portal
Survey results automatically transmit back into the chart in the Progress tab
Administering clinician-rated measures
Navigate to the patient's chart and open the Progress tab
Click the button "Administer Clinician Rated Measure or Add Score Manually"
Adding scores manually
Useful in the following situations:
You're completing the measure on behalf of a patient who can't access a phone or computer
You're transcribing responses for a patient who completed the survey with pen and paper
Frequently Asked Questions
Why does "Last Requested" date say "Never"? Are my patients not receiving their automated surveys?
The "Last Requested" date only shows when a survey was last manually requested. Surveys sent automatically are still sending if they are on.
