Business Entity Changes

Updated company name, entity, TIN, logo, address, and other changes to your practice

Updated over a week ago

If your practice business entity changes, or you change any practice information, you'll want to ensure that you update the appropriate settings to reflect those changes. Some examples of these changes to your company could include:

  • Changing your practice name

  • Changing your business entity (Tax ID Number, EIN)

  • Changing your logo

  • Changing your address

When changing this information, it is not necessary for you to open a separate Osmind account. You can maintain the same account to keep your patient information.

Practice Settings to Update

  1. My Account - Ensure all provider information (such as addresses, SSN, and EIN) is accurate. Note that providers have to access the "My Account" setting under their individual logins. These setting cannot be changed by another user.

  2. Practice Details - Update your practice name and billing information. This is also where you can change your logo.

  3. Locations - Ensure locations are updated with the correct EIN/TIN information.

  4. Customer Forms & Surveys - If you are using any custom forms, you'll want to re-upload and configure with your new business entity information.

  5. Stripe - From your Stripe Connect dashboard (can be accessed through your settings), there is a person icon in the upper right > "Platform Settings" towards the bottom. From there, you can update your account information.

  6. DrFirst - Let our support team know your new business entity or location information (including your phone and fax numbers) and we will open a ticket with DrFirst support to update that information on your account for e-prescribing.

  7. Claims - Let our support team know your new business entity or location information (including EIN/NPI numbers) and we will open a ticket with Change Healthcare support to update that information on your account.

Reach out to support@osmind.org if you have any specific questions about your scenario.
